Go to Granny’s quarters and locate her cameo on her bed pillow. Open the furnace in her chamber. As soon as you place Granny’s cameo into the furnance, she will attack you. You have to flip the furnace switch a second time to destroy the cameo, thus making mortal Granny Rags.
Can you do Granny Rags and slackjaw missions?
To get the Granny/Slackjaw confrontation in mission 7, you don’t have to complete any of the Granny or Slackjaw side quests, in mission 2. All you need to do is simply talk with Granny. That alone will trigger the confrontation later on in the Flooded District.

Just put the cameo in the furnace and burn it. Granny will appear in the room but, if you recall, she’s blind and won’t see you there. Then, just crouch (because she IS able to hear) and walk past her and hide around the corner to your left (as you pass by her).

For Granny Rags, you will have to Sleep Dart her after the cameo is destroyed (the furnace exploding) for her body to be counted as an “unconscious” body.
